Service target
Provide comprehensive and high-quality comprehensive care for the frail elderly who cannot receive family care during the day, including day care, nursing services, rehabilitation care, social care, and carer support services, so that the elderly can maintain good physical function and develop their potential and improve their quality of life, relieve the pressure faced by carers and enhance their caring skills and skills.
Service object
Elderly persons aged 60 or above living in Tuen Mun district who meet the following conditions:
stable health
Moderate to severe impairment of bodily functions
live in the community
Carers are unable to provide full-time care
not bedridden
no violent tendencies
Carers of elders using services
TOLL
Long-term service user charges are based on the criteria set by the Social Welfare Department and the number of service days per week
Additional charges apply for extended services
Respite service users are charged 41.5 yuan per day
How to apply for services
Applications should be made to district centres for the elderly, neighbourhood centres for the elderly, activity centres for the elderly, integrated domestic care services, integrated family service centres and medical social work units.
Long-term service users are subject to assessment by the Social Welfare Department Elderly Services Unified Assessment Office to determine whether the applicant is eligible for service.
Respite service users can apply directly to the service on their own or through the above-mentioned referral unit.
Eligible applicants will be arranged by the center to receive services after being assessed by the nurse.
This service will assist applicants who meet the service qualifications to enter the service according to the order of application, and negotiate the relevant service period.
Exit service procedures
Long-term service users must apply for withdrawal at least two weeks in advance and complete the procedures.
