Tuen Mun District Elderly Day Care Service Centre

Service target
Provide nursing, rehabilitation training, counseling and referral, social and recreational activities, personal care, meals, transportation, carer support and other services for the frail or disabled elders who are not cared for by their families during the day. The physical function of the elderly, strengthen the motivation and ability of the carer, and let the elderly live at home.

service object
60 years of age or older in the area, frail or physically impaired requiring care but able to ambulate with a walking aid or wheelchair, requiring personal care or basic nursing services, no acute illness or communicable disease, no recurrent symptoms of long-term mental illness, ambulatory Sick, unattended during the day, suitable for community living.

TOLL
Determined according to the charging standards set by the Social Welfare Department

How to apply for services
Applications should be made by the Family Service Centres, Medical Social Workers, District Elderly Centres or Neighbourhood Elderly Centres of SWD or non-governmental organisations. Applicants must be assessed by the Unified Assessment Office for Elderly Services of the SWD, and those who are eligible will be arranged to receive or wait for service.

Service Content 
Care Plan: Assessment, formulation, implementation and review by nurses, physiotherapists, occupational therapists and social workers for the elderly.
 

Nursing Services: Nursing services (such as medication administration and supervision), health assessments, rehabilitation guidance, and health education are provided by nurses.
 

Rehabilitation therapy: Physiotherapists and occupational therapists tailor-made rehabilitation exercise and treatment plans for the elderly, implement physical activities, provide consultation and loan services for rehabilitation equipment, and improve the mobility and physical function of the elderly.
 

Psychological and social health services: Social workers provide counseling and referral services to help elders adjust to group life and resolve emotional, financial and interpersonal problems; organize interest, development, education and volunteer groups, social and recreational activities to help elders make good use of their leisure time.
 

Daily care: provide meals; take care of the elderly for daily living, such as toileting, feeding, etc.
 

Carer support services: Organizing elderly care skills, such as post-stroke care and recovery tips, support skills, home care for the elderly with diabetes or dementia, etc.; establishing a carer support network and organizing mutual help groups.
